Question: Can the Centers for Medicare & Medicaid Services (CMS) provide any formatting guidelines for screenshots or other attachments submitted as part of the discrepancy reporting process?

Answer: Screenshots and attachments are optional for the discrepancy reporting process. Issuers should include any evidence that may assist CMS in identifying and resolving discovered discrepancies. Issuers may submit attachments in any format.
