CHIP Application Template
This draft model application template outlines the types of information that are likely
to be included in the state child health plan required under Title XXI. It has been
designed to reflect many of the requirements that will be necessary for state plans
under Title XXI. It is not intended to be comprehensive or final. We provide it for
preliminary guidance as well as to solicit additional information from states and other
interested parties on the appropriate content.
