Chronic Conditions Chartbook: 2011 Edition

Guidance for all stakeholders regarding 2011 report on demographics and prevalence, Medicare service utilization and Medicare spending for chronic conditions.

The 2011 edition of the chartbook “Chronic Conditions among Medicare Beneficiaries” was published in December 2011. The data used in this report come from the 2008 CMS administrative claims data for 100 percent of Medicare beneficiaries enrolled in the fee-for-service (FFS) program, which are available from the CMS Chronic  Condition Data Warehouse (www.ccwdata.org). The chartbook includes three sections: Section 1: Demographics and Prevalence, Section 2: Medicare Service Utilization and Section 3: Medicare Spending.

Figure 1.1b has been updated to change the data value for heart failure for the 65 and older population from 14% to 18%. The data tables and power point slides also have been updated. October 2, 2012
