Guidance for Medicare-Medicaid Plans (MMPs) in Massachusetts, Illinois, Virginia, California, and Ohio that are subject to the requirements in the memorandum, including inputting of actual mail dates in HPMS within 15 days of mailing, and the due dates of October 31, 2014 or January 15, 2015 for the submission of errata sheets to correct errors in ANOC/EOC documents.
