In 2005 the White Earth Band of the Ojibwe (White Earth) contracted with the State of Minnesota to provide case management and eligibility assessments for long term services and supports (LTSS) home- and community-based services. The program, the White Earth Long Term Care Consultation (LTCC), is tribally managed and a division of the tribal health service. It was established in 2004 to provide HCBS to White Earth tribal members, and it signed a contract with the State in 2005.
Through its contracted arrangement with Minnesota, the White Earth program administers five waivers: Elderly, 65 and older; Community Alternative Care (for hospital-level care for any individuals); Care Assistance for Disabled Individuals; Traumatic Brain Injury; and Alternative Care for Medical Assistance (state supported but provided under Elderly waiver). For those tribal members who are not approved for these waivers, tribal funds are used to obtain limited services.
As of 2011, the White Earth LTCC serves 300 clients a month through its contract with the State. The services provided by the program are administrative in nature: assistance with Medicaid application process, case management, and eligibility needs assessments. All other services are contracted out to tribal and nontribal providers. Services are not limited to American Indian and Alaska Native beneficiaries, but they are restricted to individuals who live on the reservation.
As the following table shows, the major portion of the funding revenue comes from Medicaid.

The White Earth LTCC is currently working on a contract with a managed care organization (MCO) to provide case management for those individuals signed up with the MCO. This will provide those clients living on the reservation with the option of having a tribal provider.
