Medicare Fee For Service Legacy Provider IDs Prohibited on Form CMS-1500 and Form CMS-1450 (UB-04) Claims
Medicare Fee for Service contractors shall not report Medicare legacy provider identification numbers on Form CMS-1500 and Form CMS-1450 (UB-04) claims. Claims containing legacy provider identification numbers will be returned without appeal rights. References to the former 12-90 version of the Form CMS-1500 have been deleted from the Pub. 100-04 Chapter 26, Section 10.4 manual instructions.
