Key Dates

April 3, 2014 - CMS and MI sign MOU

October 7, 2014 - CMS, MI, and participating plans execute three-way contract

March 1, 2015 - Start of opt-in enrollment in the Upper Peninsula and southwest MI

May 1, 2015 - Start of opt-in enrollment in Macomb and Wayne Counties. Passive enrollment begins in the Upper Peninsula and southwest MI

July 1, 2015 - Start of passive enrollment in Macomb and Wayne Counties

January 1, 2018 - CMS, MI and participating plans execute an amended three-way contract

January 1, 2019 - CMS, MI and participating plans execute an amended three-way contract

September 1, 2020 - CMS, MI and participating plans execute an amended three-way contract

January 1, 2022 - CMS, MI and participating plans execute an updated three-way contract & an amendment to the three-way contract

Health Link Model

On April 3, 2014, the Centers for Medicare & Medicaid Services (CMS) announced a new partnership with the State of Michigan to test an initiative to improve care for Medicare-Medicaid enrollees.

Under the demonstration, Michigan and CMS will contract with health plans called Integrated Care Organizations (ICOs), which will provide an integrated set of Medicare and Medicaid benefits to enrolled beneficiaries in four regions of the state.
