MLN Product (Revised)- Creating an Effective Hospice Plan of Care

This revised product comprises Subregulatory Guidance for creating and coordinating successful hospice Plan of Care (POC), and its content is based on publically available content within at https://ecfr.io/Title-42/Section-418.56, https://www.federalregister.gov/documents/2020/08/04/2020-16991/medicare-program-fy-2021-hospice-wage-index-and-payment-rate-update and https://www.cms.gov/Regulations-and-Guidance/Legislation/CFCsAndCoPs/Hospice. CMS policy or operation subject matter experts also reviewed/cleared this product. This product educates on the roles of the Interdisciplinary Group (IDG) and RN Coordinator, principles of quality care planning and common deficiencies related to POC implementation.
Revisions to this product didn’t include any substantive content updates. The previous version of this product is available to the public and currently posted on the CMS website at https://www.cms.gov/files/document/creating-effective-hospice-plan-care.pdf.
