Mobility Action Group Change Package and Toolkit
Guidance for the CMS Center for Medicare and Medicaid Innovation (CMMI) in partnership with the Comprehensive Care for Joint Replacement (CJR) model and Bundled Payments for Care Improvement (BPCI) Initiative Learning System Teams have created a Mobility Action Group Change Package and Toolkit based on findings from the Mobility Action Group in which participants in the BPCI and CJR models participated. The purpose of the toolkit is to provide a conceptual framework for change and an implementation roadmap that hospitals can use to increase mobility among older, hospitalized adults. Please note that the framework and concepts provided in this change package are not part of any requirements to participate in either model; nor is this toolkit meant to serve as a formal guideline. We understand that not all of the concepts discussed in this toolkit will be applicable to all model participants. We hope that this toolkit will help in your efforts to enhance patient mobility within your organization.
