Modification to Existing Medicare Summary Notice (MSN) Procedures Regarding the MSN Customer Service Information Box, Beneficiary Estate Information and the Appeals Address

This change request clarifies the requirements for FIs and carriers, as well as Medicare administrative contractors (MACs) and durable medical equipment Medicare administrative contractors (DME MACs), with regards to changes to the Customer Service Information Box of the Medicare Summary Notice. This is a correction to Business Requirement 5889.3, issued under Transmittal 1444. We modified this requirement to allow FIs and carriers to include a message in the Customer Service Box referring beneficiaries to the General information Section of the MSN for the appeals address, since they do not have the capability to print their appeals address in the appeals section of the MSN. MACs and DME MACs do not need to include this message in the Customer Service Box as they are required to include their appeals address in the appeals section.
