Removed all UB-92 version 6.0 record types information (section 90). New Occurrence Code "DR" has been added. New Occurrence Span Code "MR" has been added. Value Code 61's definition has been updated. Value codes Y1, Y2, Y3, and Y4 have been added. Revenue codes 0521, 0522, 0524, 0525, 0527, and 0528 have been added/updated.
