Part D Prescriber Enrollment Information

Guidance for Medicare providers with information on the preclusion list.

In a final rule published on April 16, 2018 (CMS-4182-F), CMS rescinded the requirement that a provider be enrolled in Medicare in order for their prescriptions to be coverable under the Medicare Part D program beginning 2019. In the same rule, CMS implemented as an alternative that the provider must not be on a preclusion list in order for the drug to be coverable under the Medicare Part D program.
