Guidance that rescinds Transmittal 2510, dated August 3, 2012, and replaces it with Transmittal 2574 dated October 26, 2012. Chapter 4, section 250.16 of the Medicare Claim Processing Manual was changed to section 250.17 to add a new section. All other information remains the same.
