Update of Internet Only Manual (IOM), Pub. 100-04, Chapter 8 - Outpatient ESRD Hospital, Independent Facility, and Physician/Supplier Claims
The purpose of this Change Request (CR) is to update the title of chapter 8, section 50 and to update chapter 8, section 50.3 to insert existing condition codes related to home dialysis that were previously removed in error.
